Background:

The Pakistan in-country immunization supply chain (iSC) manages about $151 million worth of vaccines every year through an immunization supply chain structure that flows from the Central (Primary) store in Islamabad to four provinces (Punjab, Sindh, Khyber Pakhtunkhwa and Balochistan) and three Federally administered areas of Azad Jammu & Kashmir, Gilgit-Baltistan, and Islamabad, Districts and/or Tehsils and Service Delivery Points.

The 2014 iSC EVMA for Pakistan showed an aggregate score of 61% and stakeholders conceptualized four strategic goals with the aim of achieving an 80% milestone for all EVM criteria by 2020. 

Using EPI partners and stakeholders workshops in October 2017 for system design concept introduction and scenario finalization and modelling workshop in August 2017, the system design analysis in Pakistan was structured into two parts- a point of entry analysis (analyzing alternative scenarios to all vaccines being received in Islamabad including to all Provinces) and a Provincial analysis (including Provincial- specific alternative scenarios to current iSC system; using different cost, efficiency and equity metrics.

On Cost, results for the point of entry analysis show that In Sindh, there is potential savings of about $1m (PKR100M) in vaccine (RI and Polio) freight costs and about $500k (PKR50M) in dry goods freight costs over 5 years; while in Punjab, KP and Balochistan, changes in point of entry increases vaccine (RI and Polio) freight costs by $700k(PRK85M), $215 (PRK24M), $30k (PRK3.5M) respectively. 

On Efficiency, for point of entry analysis reflects potential distance travelled by vaccines by 99% on $38M(PKR4bn) worth of vaccines (Sindh), 81% on $74M(PKR8bn) worth of vaccines (Punjab) and 83% on $8M (PKR924M) worth of vaccines (Balochistan); and potential reduction in distance travelled of dry goods by 99.7% (Sindh), 81% (Punjab) and 83% (Balochistan).

2.0 Project details of the SYSTEM Design IMPLEMENTATION COSTING ANALYSIS

2.1. Objectives of the System Design Implementation Costing Analysis

The system design implementation costing analysis aims to:

  • Produce a framework (using the roadmap) for analyzing the different cost implications of implementing the system design analysis recommendations.
  • Conduct a detailed costing (capital, operational, recurrent etc.) analysis (using developed framework) of implementing the system design analysis.
  • Share initial results of costing analysis with in-country stakeholders for feedback and recommendations.
  • Based on feedback and recommendations of in-country stakeholders and others, classify the financing needs into immediate, medium and long-term.
  • Produce a report of the results of system design implementation costing analysis for implementation with them. 

2.2 Methodology

The methodology that shall be employed in delivery this service shall include

  • Desk review of implementation roadmap report.
  • Visit each province & Areas to agree on actual activities to implement and develop timelines for implementation with them.
  • Use the information above to develop a costed implementation plan.
  • Employ any other method that would improve quality of deliverables.

Task to be Performed

  • Produce a framework (using the existing roadmap) for analyzing the different cost implications for implementing the system design changes for each Province/Areas with recommendations after consultative meeting with them.
  • Conduct a detailed costing (capital, operational, recurrent etc.) analysis (using developed framework) for implementing the system design analysis and share initial results of costing analysis with in-country stakeholders for feedback and recommendations.
  • Based consultative discussions with key stakeholders on feedback and recommendations of in-country stakeholders, classify the financing needs into immediate, medium and long-term and produce a final report of the results of system design implementation costing analysis.

Deliverables:

D.1 Share framework and report on outcome agreed by each Province/Areas.

D.2 Draft costing analysis report with recommendations.

D.3 Final report of the results of system design implementation indicating agreed framework, costing analysis and timelines.

3 months duration from start date

To qualify as an advocate for every child you will have…

Qualifications or Specialized Knowledge/Experience Required AND Competencies

Candidates should possess a first degree in any area of sciences, engineering and business studies. A higher qualification in any of this field would be an advantage.

Candidates should have a minimum of 8 to 10 years experience in immunization supply chain management,

Should have at least have 5 years of professional experience at national and international level in EPI logistics management (Cold Chain, Vaccine Management, Immunization Supply chain, immunization service delivery), a good EVM field experience.

An understanding of system design as a supply chain fundamental.

A good knowledge of translating activities into costed implementation plans would be a strong advantage.
